Ensuring someone has a place they can call home is both a basic human right and a key component of effective case management. If a person does not have access to appropriate, safe and secure housing it may be difficult to help them achieve their care plan goals. This section allows people to talk about their current housing situation, their experience in relation to past accommodation and also any past experience of homelessness. It is important for a Key Worker to know this information so they can refer someone to appropriate accommodation/housing and support services if necessary.
